Group A strep will release antigens (Nephritis-associated plasmin receptor and Streptococcal pyrogenic exotoxin B) that IgG & IgM antibodies will then bind to and form immune complexes. These will then insert themselves into the wall of the glomerular capillary. Immune complexes activate the complement system which brings in neutrophils with bind to receptors on the immune complex to release proteases, reactive oxygen species and neutrophil extracellular traps which damage the capillary wall. Macrophages enter and cytokines exacerbate the capillary damage. Mesangial cells with proliferate and cause expansion. This will all cumulate in hematuria, proteinuria, edema and hypertension.
