Tom works at a factory that manufactures heavy machinery. His job involves operating large equipment, including cranes and forklifts. Tom has been working at the factory for a few months and has received some on-the-job training from his supervisor, Jack. However, he has not received any formal training on the safe operation of the equipment. One day, while operating a forklift, Tom accidentally hits a stack of boxes, causing them to fall and injure another employee. The injured employee is rushed to the hospital with a broken arm and several bruises. An investigation reveals that Tom was not properly trained on the safe operation of the forklift.

What are Tom's obligations under OSHA 1994 regarding his own safety and the safety of his colleagues while operating heavy machinery?

As Tom's supervisor, what are Jack's responsibilities under OSHA 1994 concerning the training and supervision of employees operating heavy machinery?

Under Malaysian safety standards, discuss whether "on-the-job training" from a supervisor is a sufficient substitute for formal competency certification.
