The Creation Of A Black Hole: A black hole is formed when a very massive star exhausts its nuclear fuel, its core collapses under its own gravity and that creates a black hole. But the collapse often results in a Supernova that blasts some of the stars material into space.

Why have we not been in a black hole yet?

Because the gravity of a black hole is extremely strong that if a humanoid or anything in fact were to get close to a black hole the gravity of it would suck you in and as you are floating through this vast dark abyss of a place minutes become hours until finally your just about to reach the core (Singularity) right before you reach the future itself time itself right before all your questions are about to be answered spaghettification happens it happens when the gravity of something is so strong it pulls this person/thing and stretches them over and over again in every way possible until you are ripped to nonexistent particles.

Why have we not been in a black hole yet?

Because not only is it impossible but you would get instantly ripped apart from the gravity into dust particles then nothing, that is the power of a black hole, black holes have the ability to swallow planets. The biggest black hole on file can fit up to 100 billion of are sun's.
