         <description><![CDATA[<div>I will inform Tan that based on the research the adult patient and nurses both prefer handover at the bedside rather than somewhere else.<br>I will inform Tan that patients find the bedside handover beneficial as they feel they are involved in their own care and supports two-way communication.<br>I will advise Tan that patients prefer to have a family member, carer or friend and 2 nurses instead of nursing team during bedside handover however having a family member, carer or a friend was considered not important by nurse's side.&nbsp;<br>I will explain to Tan, that the patient agreed for sensitive information to be handed over quietly in the bedside but was strongly disagreed by nurses who preferred handing the sensitive information over verbally away from bedside.<br>I will advise Tan that the implementation of important patient-centred safety could be improved by developing the design and process of bedside handover.&nbsp;<br><br><br><br></div>]]></description>

         <description><![CDATA[<div>•&nbsp; I will explain briefly in regards to necessity and article summary and instructions of practice which aid in to cervical Screening<br>• I will explain the causes and causative organism which involves in the health condition which is HPV.<br>•I will encourage Roshni to get the test done as early as possible,to identify the clinical manifestations of HPV to find the risk of cervical cancer which will take to develop in time period of 10 to 20 years in future.<br>•I will provide the awareness regarding the cervical cancer which will be peaker in the middle age of life with who is having cervix.<br>•I will deliver the attention to the concern of Roshini by providing psychological support, family involvement, adequate informations and other non pharmacological measures to relieve the anxiety.<br>•I will advice Roshini in regards to attend the screening according to the time period that is, 3 years until she turns 49 and every 5 years until advised to do so and after 65 years either no screening or need to attend the screening if there is evidence of abnormal previous result.</div>]]></description>

However, it is also safe to administer Ibuprofen without food for short term (up to 1 week).<br>• I will remind Selai  that when the child has asthma, both Paracetamol and Ibuprofen can worsen the respiratory symptoms.<br>• I will inform Selai that in general, both Paracetamol and Ibuprofen is safe tp be given to children to treat fever.<br>• I will note that it is perceived for Ibuprofen to create more side effects than Paracetamol but it has not been proven yet as they still lack evidence to support this claim.</div>]]></description>
