Since I was a little girl, I have always felt the need to help others with my actions. I have always been a selfless child, always active in volunteering and in the assistance to the poor, desirous of being a change in the world. When I grew up, however, I realized that I couldn’t change the whole world but I could start by changing the world of individual people. So, after high school, I chose to dedicate my life to nursing because I wanted to undertake a job that would gratify me at the end of the day and that could appease my desire to help people in difficulty, especially children. Although my parents did not agree, I followed my heart that led me to do a job in which there is a continuous exchange of love with the patient. The nurse is the one who dedicates his life without expecting anything in return. Although it is a demanding and stressful job, I love seeing children who smile at me and hug me just for helping them in something seemingly trivial. I want to become a nurse because I want to accompany those children in the most difficult moments of their lives and to see them enjoy another chance after so much pain.

The TV Show tells the story of Professor Andrea Fanti, a brilliant doctor who works in Milan at Policlinico Ambrosiano where he plays the role of chief of Internal medicine. Fanti is an arrogant doctor who does not believe in the doctor-patient relationship because he thinks that all patients lie. His life is upset when the father of a patient who died in the ward, due to a mistake by the medical team, shoots him in the head. Andrea survives the shot but suffers an irreversible pre-frontal injury. The trauma causes him to lose the memory of the last twelve years and so, after his awakening, Fanti is convinced that it is June 2008 instead of 2020.

The recording concerns a nurse student who is receiving instructions from a paramedic to implement the cardiopulmonary resuscitation. Initially, the paramedic asks the student to check the pulse to examine the patient ’s state of consciousness. She replies that the patient has no pulse and he is not breathing. So the paramedic claims that the patient has had a cardiac arrest and tells the nurse to give CPR. She has to support his head and to hold his nose close and his mouth open. The nurse gives two breaths into the patient’s mouth and checks the pulse. Since nothing happens, the paramedic recommends the nurse to turn patient’s head and to put one of her hands on his chest and the other one on the top of her first hand. She has to push down a little and to release. The paramedic counts up to four after each push down on the chest. The nurse repeats the procedure for fifteen times. After doing that, she checks again the pulse but it is still undetectable. So the paramedic suggests to use the defibrillator. They set the charge on it at 200. The nurse applies the pads on the chest above and below the heart, makes sure that she does not touch the body, calls “everybody clear out” and presses the bottom for two seconds. The patient starts to respond after the second charge from the defibrillator. So the paramedic tells the nurse to give the patient lidocaine. The dosage is 100 ml over two minutes.

ANAESTHESIA

Anaesthesia is a “loss of sensation” caused by Drugs that work by blocking the signals that pass along the nerves to the brain. This drugs, called Anaesthetics, stop the feeling of pain. When the Anaesthetics wear off, the patient starts to feel normal sensations again. The development of effective anaesthetics in the 19th century was an important factor in successful surgery. Before this time, some doctors used alcohol and morphine to reduce pain. Few operations were possible and surgeons were judged by their speed. Many patients died on the operating table. When Anaesthesia was born, surgeons could take more time and perform more complex procedures. One of earliest anaesthetics was Ether but it had some drawbacks- for example, it could cause vomiting. It was quickly replaced by Chloroform, which was more potent and easier to use than ether. However, it was not as safe to use as ether because it could cause sudden death. By the 1920s, intravenous induction agents were introduced. In the 1940s muscle relaxants became available. Anaesthesia can be given in different ways and not all anaesthesia makes unconscious:

Local anaesthesia numbs a small part of the body and the patient is conscious but free from pain;

Regional anaesthesia can be used for operations on larger or deeper parts of the body and the patient stays conscious but free from pain. The most common regional anaesthetics are spinal and epidural anaesthetics used for operations on the lower body;

General anaesthesia is a state of controlled unconsciousness and the patient feels nothing. It is used in abdominal surgery.
Now Anaesthesia is very safe due to modern monitoring systems and a greater understanding of the functions of the body. Death during operations is not resulting common from anaesthesia.

A good nurse is one who can put into practice what he has studied assuming, at the same time, an empathetic attitude towards patients. He works with the utmost professionalism showing humanity and respect for others. A good nurse identifies with the patient trying to establish a relationship of mutual trust so that the patient can turn to him if necessary. A nurse has to be an organized person, open to listening and group work. He has to be able to confront patients, colleagues and doctors so that effective healthcare can be achieved. He has to be able to welcome the fears of patients and their families by offering support and comfort. He has to transmit and receive information calmly and quickly. In addition, a nurse must act in critical situations necessarily with the utmost calm, remaining professional and able to play his role. I believe that in future I could be a good nurse because I am a very empathetic and human person. I would like to support my patients in every moment of discomfort and rejoice with them for every little victory. I think I am a very rational person who can act clearly even in the most difficult situations. I would like to help those who are in a time of suffering and make a difference in their lives even with a simple gesture of kindness.

I imagine working in a very large public hospital characterized by numerous specialized and surgical departments capable of treating a wide variety of pathologies. It will be a non-profit hospital, based on contributions from volunteers, that deals with scientific research and is committed to ensuring the well-being of patients by providing the maximum form of assistance with the use of advanced medical equipment and with the choice of excellent healthcare personnel. I would like to become an emergency nurse with a developed ability to understand with a glance if there is a need to intervene immediately on the child or i would like to become an oncology nurse able to listen to the needs and desires of the patients, to support them in times of suffering and accompany them in the end of life.
