         <description><![CDATA[<div>You are the supervisor during a deployment exercise which involves a commander and three crew who were hooking up equipment to the vehicles.&nbsp;<br>The men are working by the torchlight in the dark of night. Due to a lapse in concentration, the hook point was not secured, causing the equipment to drop to the ground.<br>The commander managed to recover the equipment, but kept quiet about the incident instead of reporting it to you.
