I'm a fan of exit tickets/exit passes right now! I ask students to answer a small number of content-based questions for the last 5 minutes of the lesson. Most of these exit tickets are in Google Forms, so it's easy to review the students' answers at any time. Depending on the type of question, Google Forms will even compile the students' answers for you. I also write the content area and date in the title of each form so the data stays organized.

I like formative assessments in music that tend to be quick and embedded into the teaching process. Interactive modeling is helpful when I want students to focus on a certain process or technique. We sing solfege with hand signs; students identify notes on the staff and show with hand signs. We play listening games to assess aural identification of rhythmic/melodic patterns. Students self-evaluate performances/check for understanding with a quick thumbs up/down/middle scale. They peer-evaluate using a rubric and scale of 1-4 (Novice to Master).

I like using simple problems and stamping of independent work to ensure that students have understood whatever we have been working on. It helps me help students and see what they understand and don't understand about what we are working on. It is a bit more time consuming to check everyone's work, but it's well worth it. Of course, I also ask students to double check their work when they have gotten an answer wrong.

For math I have them write their own equations with their own drawings to see who is understanding the concept. I use thumbs up and thumbs down too.

I also have students do work on their white boards (especially in math) and show me their answer by holding it against their chest. I then record students who are struggling with the certain concept and go back and help them. I also have students self monitor and double check their work.
