Unlike the U.S, Japan’s health care system is publicly administered. As of 1958 one hundred percent of residents are covered by universal healthcare.

Japan is made up of four main islands Honshu, Hokkaido, kyushu and Shikoku and home to the worlds third largest economy. The Japanese have one of the lowest fertility rates in the world and simultaneously one of the longest life expectancies. In Japan, policymakers traditionally expected the country’s younger generation to care for their aging parents however, Japan’s family-centered approach failed. The primary caregivers — grew overwhelmed by the task, especially with the trend toward fewer children and more women joining the workforce. Public pressure propelled reform and Japan came up with a public, mandatory long-term care insurance system in 2000

An Aging Population

A concern both nations have in common is how to deal with their aging population and the demands in which it has on the healthcare system. In the U.S., persons 65 years or older represented 14.5% of the population in 2014, but are expected to grow to be 21.7% of the population by 2040. (U.S. Department of Health and Human Resources,2016). These individuals are at high risk for complex health problems, chronic illness, and disability, and they are, and will continue to be, the heaviest users of health care.

Medicare is a national insurance program, administered by the U.S. Federal government. It provides health insurance for Americans aged 65 and older, as well as some younger people with disabilities. Medicare consists of three major categories of coverage. Medicare Part A, Hospital Insurance, helps cover inpatient care in hospitals, including critical access hospitals, and skilled nursing facilities . It also helps cover hospice care and some home health care. What Medicare does not cover is Long term Care. Long term care includes medical and non medical care provided to people who are unable to perform basic activities of daily living, like dressing or bathing.

What we can learn from Japan ...

Despite its success, Japan’s health and long-term-care systems face similar sustainability issues as the United States, including rising costs and increasing demand. However Japan has succeeded at keeping its people healthy and in their communities for as long as possible. As of 50 years ago Medicare had covered Americans 100%, unfortunately coverage has decreased as well as being centered on acute care rather than long term care. Japan’s initiative to help at home care takers is one that American’s would be happy to see here. Although neither country has a perfect system Japan has been able to more readily adapt to its aging population and the economical and even personal effects it can have on its people.
