There are many different answers to this question. After all, it is one that has haunted the society of 'thinking questions' for a very long time. Some of the answers might be a celebrity of the time, or, more recently, a cell phone. The latter seems a little ridiculous, because on a deserted island there is little chance of a working cellular tower in the area. Some other answers would be things such as a boat, or a plane. However, these answers violate the whole idea of being stuck on an island, so my answer will not be one of these. Instead, I will direct my answer to survival on the island.
The first of my items would be a water purifier that does not require batteries or energy to run, because, if the island is surrounded by ocean, then one must have means to get water, or one would die on the third day. The second item that I would require would be a small plot of land that would be good for planting, seeds included. Different types of seeds, so I could circulate crops, so the ground is never too devoid of essential minerals. The third would be a knife of some sort, as they are useful both for protection and construction. With a knife I could do many things, including, possibly, hunt, assuming that the island is large enough to have animal life. The fourth, I would say would be company. Ideally my sister. But, perhaps this is incredibly selfish of me, to drag her onto this hypothetical island when she could be safe at home. Still, the selfish part of me wishes that my sister would be on the island with me so we could (eventually) grow crazy and old on the island together. The unselfish part would ask, instead, for a house or shelter of some sort, something durable enough to last all weathers.
This is, of course, all assuming that, by the laws of making this a difficult question, I could not ask for a boat, a navigator, a map, and sufficient supplies to last us our journey.
