Trusted Venue Ambassador engages with the resident and learns what is needed. Resident confidentiality must be maintained.

With the resident’s permission, the Ambassador submits the Healthcare Referral online form to the Coalition Point of Contact (CPOC), when necessary.

The CPOC refers the resident to the appropriate Community Health Worker (CHW).

The CHW connects the resident with care or addresses the need. CHWs follow up with residents to ensure a closed loop.

CHWs will collect basic demographic information in order to track volume and types of help needed. Resident confidentiality will be maintained.

Ambassadors will meet regularly for networking and information sharing.

Formal CHW trainings will be coordinated in the near future. Ambassadors who are interested in CHW training will be invited to participate.
