A speech evaluation report is all about providing information on a child's language and communications skills. Each section of the report documents the child's history and how they scored in each section of the speech-language evaluation. The overall goal of speech evaluation form is to determine whether the child requires speech therapy or any accommodations. Each section within an evaluation report provides the information needed for the evaluation to be effective, accurate, and helpful.

From the examples that were viewed of speech evaluation reports there does not seem to be much room for variation. The reports seem to have a required setup with specific sections. The most variation I found is in how the evaluation results are presented. For example, in some reports the written evaluation and the scores are all in the same section (more in a rubric form), but in the others the test scores are placed in a separate section at the end (the example provided above).

Speech evaluation reports are helpful tools for creating a cohesive view on a child's background, speech and communication development, and what the child needs to be successful in building their communication skills.

Though some potential problems are that each evaluator may have a different opinion on where the child falls on the development scale for each skill, causing confusion for the parents and an ineffective plan of action on how to build the child's language development skills.
