DEADLINE EXTENDED FOR GUEST FORMS! CLOSING OCTOBER 8TH @ 5:00 PM

NO FORMS ACCEPTED AFTER CLOSE.

SUBMIT COMPLETE DANCE GUEST FORMS AND PHOTO IDENTIFICATION FOR PRE-APPROVAL.

ALL NON-BHS GUESTS MUST RECEIVE PRE-APPROVAL FOR TICKET PURCHASE.

NO ENTRY AND NO REFUNDS FOR NON-APPROVED GUESTS.

To get your Non-BHS guest approved follow these steps in order:

1) Download the BHS Dance Guest Clearance Form from https://padlet.com/e010276_1/bhshoco2025

2) Send the form to the Guest’s School and Guardian for completion.

3) Obtain Completed Form and Photo Identification for the guest.

4) Scan and Label files with BHS Student’s First and Last Name (I.E. ChristySohn-GuestForm or ChristySohn-PhotoID)

5) Upload files to Microsoft Form Link on https://padlet.com/e010276_1/bhshoco2025

6) Receive email confirmation of approval from Christina.Sohn@stjohns.k12.fl.us

7) Purchase tickets via GoFan.co or Cash/Check before October 9 at noon.

Ticket Sales Close October 9 at Noon

Cash/Check Tickets will be available October 7, 8, & 9 from 8:30 - 9:20 am in the BHS Courtyard only.

Do not purchase Guest Tickets without Pre-Approval.
