Many times teachers can provide time during prep/before/after school but it cannot work with that teacher. 
Some students are not willing, time related or not.

Larger classes can make the time that teachers spend with students incredibly low.

If there are conflicts before or after school, it is difficult then too.

They're given the opportunity, but sometimes it's their responsibility to come in for that extra help, and that "guarantee" has to come from them.
