When do we need to complete the LD form?<br><br>-Will get clarification on when to complete form<br><br>CLARIFICATION:<br>Only when suspecting LD<br><br><br>Sara Alberti said we always have to fill out the LD checklist as a rule out and reference this in the PPT itself.  Do this if there is any question of it at all; it’s a safety net.  It is documentation that we went through the process and we considered it.  Without the form being completed there is no “evidence”.  Filling out the LD checklist is not a legal requirement but rather best practice.
