Low barrier entry tasks, relevant instruction, and lots of time for student activity are key here. If a classroom has positive momentum, it is more difficult to be off task 
-When a student is off task, I often set the class to working on something and I address the student one on one and quietly to discover what's up. For instance, if a student is  sleepy, there might be a very justified reason why this is the case.
