The ladder to be secured with regular and 3 extra lashing

When changing wire all sheaves and nipples to be cleared,cleaned

All limits and interlocks to be checked, greased and tried out.

Accommodation ladder wire renewal

During carrying out the job take 3 additional lashing strong points

Greasing of all hidden and wire while replacing

Tool box meeting after renewal of wire and before testing

Complete testing after renewal

Before & after completion of job inform bridge

After completion of renewal and testing securing has be to be done for sea

1. Winter precaution

2. RA and work aloft permit

3. Ensure Familiarisation of new crew and all crew involved

4. Cross verification of spare availability and grease applicable

5. Availablity of winter gear

6. Timely reporting to bridge and ch off

7. Extra lashing atleast 3 to be taken

8. Sub zero condition precaution

9. Checking of limit switch, sheaves and unaccessible parts

10. Familiarise from maker manual about the wire rope specifications

11. After completion of job, inspection and try the equipment for correct operation and prepare closure report

Four new joiners bosun 1 ab and 1 os

Chief officer instructed bosun to change wire and take before and after photographs of all the sheeves and wires.

Correct lowering procedure to be followed and wire to be fully greased prior and during coiling on the drum.

Tool box meeting needed before the test lowering of the ladder and make sure secured after wards for sea.

Chief officer asked bosun to risk assessment in pal and use correct ppe for cold weather

Three more additional lashings to be used along the length of the ladder to strong points

Weather is zero degrees and beam swells.

During job all sheeves linit switches and inaccessible areas to be checked and greased

Master refuse to give permission as weather is not good , will wait for weather improvement
