A sincere apology must be preceded by reflection on the wrongdoing. An apology without reflection is just a list of words. Even if there is reflection, it does not necessarily mean that an apology is made. This is because even if they realize that they are wrong and reflect on it, they choose denial and concealment instead of an apology. Acknowledge the mistake, reflect on it, apologize, and finally, action must follow. Action is keeping the promise contained in the apology, the promise not to make a mistake again. This to me also includes actually showing that you meant what you said and not just repeating the same mistake.
