This program offers individuals with intellectual and developmental disabilities opportunities to explore leisure interest (e.g., arts, storytelling, film, music, adaptive gardening etc. This is one of several programs offered at the Pearl Buck Center. Eligibility for services aligns with the requirements for the Developmental Disabilities Council. Many clients are referred for services or apply for services at a young age and placed with this organization.&nbsp;</div>]]></description>

They support individuals in finding competitive Integrated Employment in their communities. To be eligible for services AWC must receive a referral through Vocational Rehabilitation. Once the referral has been made AWC staff work with client to complete a discovery process to identify interest, strengths, needed supports, and ultimately a job placement.&nbsp;</div>]]></description>

The Whole Person focuses on individual advocacy and people independently making decisions regarding their life with counseling from the Whole Person. They provide many workshops open to the public on topics in Independent Living. The Whole Person is Kansas City's Center for Independent Living and focuses on peer support, information and referral, individual and systems advocacy, independent living skills training, and transitions. All services are free, but an open case with and referral from Vocational Rehabilitation is needed to access individualized services and programming.&nbsp;</div>]]></description>

They are known to have a one of the largest and most respected on-site assisted living facilities in the area. Catering to adults with a wide variety of disabilities, they have one large group-home setting with several dozen smaller, more independent style living quarters around the parameter of the property. While they do offer assisted employment services, Holy Angels is most known for their supported living services and facilities.          <description><![CDATA[<div>Where Holy Angels provides assisted living for those with cognitive and physical disabilities, Evergreen Life Services has had a tremendous impact on the surrounding community by providing assisted employment opportunities for the same population. Evergreen has a large footprint in this region with several locations throughout Northwest Louisiana. While they do have assisted and independent living programs, Evergreen has had the most success providing supports and employment opportunities along with vocational and transitional services tailored to adults with disabilities. They have also done a great job at raising community awareness and support in regards to the needs of our disabled neighbors in this area. Job seekers are able to navigate the database which is maintained with opportunities from a network of employers that place an emphasis on disability inclusion.  Ability Links also hosts virtual job fairs and maintains an online bank of educational resources.  Job seekers must be at least eighteen years of age and have a diagnosed disability.</div>]]></description>

They strive for employment and retention for all clients.  The website description sums up Challenge Unlimited as a Commission on Accreditation of Rehabilitation Facilities (CARF) accredited Community Rehabilitation Program (CRP) provider, Challenge Unlimited fulfills its mission through adult and youth programs, work skills training centers, employment services, and community-integrated living options.          <description><![CDATA[<div>This organization opened in 1984 as counseling support for individuals with intellectual and physical disabilities, and traumatic brain injury.  It quickly grew in scope and became the first group in Missouri to offer individual supportive living services.  They provide (if necessary) 24 hour support in an apartment or house; counseling and behavior therapy; part time community support, and an extensive training program for CNA's and managers working for the program.  The community support provides the client the opportunity to develop more independent living skills, become involved in the community, and increase self-determination.  The support categories are health, finances, housing, maintenance, transportation, recreation, etc.  They serve St. Louis and have 70 other communities who utilize their service.  They also provide an arts programs for all ages and abilities. </div>]]></description>

         <description><![CDATA[<div>Sunflower Bakery is a non-profit organization that prepares young adults with learning differences for employment in baking, pastry, and food related industries. The program supports adults 18 years or older with a designated support person (e.g., teacher, family member, service provider) the opportunity to receive a paid internship through their Pastry Arts Training. Individuals receive 240 hours of formal pastry instruction, comprehensive employment development, 160 hours of on-the-job training, and 160 hours in a paid internship in the production kitchen. Interns also complete a rotation in the sister cafe, Cafe Sunflower, where they learn customer service skills, sales, and retail operations. The tuition is $5,850 plus an application and assessment fee. Individuals receiving funding from Maryland Division of Rehabilitation Services (DORS) may be approved to receive training here. <br><br></div>]]></description>

After joining Employment Plus, the individual identified with having a disability (ties) works with staff to find an internship that best suits him/her. Once the internship is completed, the agency (Employment Plus through Horizon House) will help the individual find a satisfying job. Employment Plus will partner with the individual to find and keep a job, identify and build on the individual's strengths as well as help the individual explore interests and career options.          <description><![CDATA[<div> Right in the Community has 23 group homes, providing year-round supervised residence for adults with developmental disabilities. Our homes are attractive, affordable and have ADA-approved accessibility features, such as wide halls and doorways, roll-in showers, and reduced-height countertops that enable people with physical and mental disabilities to live as barrier-free as possible. To live in one of our homes, you must have NOW/COMP Program Medicaid Waiver and have a diagnosed developmental disability. They also respite care that is a period of relief for families who have ongoing caregiving responsibilities for a child or adult with disabilities. Respite care can take place at the family home, in a respite caregiver’s home, a community respite home or a daycare program. Respite care can also be a natural outgrowth of caring and sharing between friends, neighbors, church groups and others, or a part-time job through a paid respite caregiving program.<br><br></div>]]></description>

         <description><![CDATA[<div> The goal of Tommy Nobis Center is to provide meaningful work for all individuals with disabilities who want to work. By creating a system built on understand and support, we change lives daily through employment and vocational services. Their employment services are individualized for each participate based on a person’s interest, abilities, needs and vocational goals. Services may include vocational evaluation to identify strengths and needs, followed by classes in personal budgeting, communication skills, safety and arranging transportation to work. Tommy Nobis Center is dedicated to empowering individuals with tools they need to obtain meaningful employment. The real-work programs helps an individual develop the necessary physical capacities, proper work habits and skills, and social interpersonal skills to find and keep a job. The service is provided at Tommy Nobis Center or at a community based company such as WellStar warehouse, Kroger, Owens &amp; Minor.<br><br></div>]]></description>

         <description><![CDATA[<div>Ability Counts advocates and promotes a partnership with people with developmental disabilities for the purpose of them to gain access to quality vocational skills, vocational choices, appropriate competitive employment, and social integration. They have "Work Activity Programs" which is a euphemism for sheltered workshops. They also have "Supported Work Opportunities" which work very similar to my in-school program with the students going out in small group enclaves. They also have individual placement services. </div>]]></description>

         <description><![CDATA[<div>Project Search helps students with significant intellectual and developmental disabilities transition from school to employment. They work at various internships throughout the community and will often be offered paid employment if they successfully complete the program. To be eligible for the program students must have an intellectual or developmental disability and be in their last year of high school eligibility, but the program can be adapted for young adults who and not currently enrolled in school. This is the first year Project Search is in our community and is housed at Mercyhealth Hospital in Rockford, IL.</div>]]></description>

         <description><![CDATA[<div> </div><div>Ken's Krew provides opportunity for young adults with intellectual and developmental disabilities the opportunity to access jobs and training for sustained employment.  Ken's Krew was created by Ken Langone, who funded Arthur Blank and Bernie Marcus when they began Home Depot. The goal of Ken's Krew is for individuals with disabilities to gain independence, confidence and become productive members of society through meaningful employment. In order to be eligible for Ken's Krew, one must be at least 18 years of age, have a documented intellectual or developmental disability, have a desire to work, have family support, submit to testing as required, and be responsible for transportation. Additionally applicants must meet specific employability, physical, and social/behavioral standards. An applicant's eligibility is based on educator and/or transition coordinator evaluations, student interview and observation, parent/guardian meeting, and onsite employment assessment. Ken's Krew provides more support than many agencies. They provide one-on-one skills assessment, job matching, vocational training and continuous support to allow individuals to work at their highest level in jobs at Home Depot and CVS. Ken's Krew job coaches provide intense training for candidates for approximately 6-8 weeks. When individuals are ready, job coaches begin phasing away from participant, but are available for retraining and support for as long as a Ken’s Krew corporate partners employ the participant. Ken's Krew is a separate entity, and not a VR vendor. Applicants can directly apply to the program. Ken's Krew is currently in New York, New Jersey, Pennsylvania, Delaware, Maryland, Georgia, and Florida. <br><br></div>]]></description>

Some of the services they offer are locate and secure affordable housing, arrange for home remodeling to ensure independent safe functioning, and provide assistive equipment and devices among many others. All people with disabilities can receive some level of support from RAMP, but there are certain stricter criteria for some specific programs that RAMP-CIL provides.</div>]]></description>

         <description><![CDATA[<div>Champions Place is a new development in metro Atlanta developed specifically for young adults with physical disabilities, some who will need support and others who can live independently. Champions Place is being developed by the Champions Community Foundation, Inc., which is a 501 C3 nonprofit organization. This shared living, fully wheelchair accessible community will provide a cost effective independent living opportunity for 16 individuals, with the plan to expand in the future. Residents must be a minimum age of 21 years, participate in fund-raising with their families and use the Medicaid Waiver to pay for services. Champions Place began accepting applications in August of 2018.  Champions Place allows for an interactive environment, with the opportunity to work and volunteer in the local community while also participating in social and recreation activities, including wheelchair athletics. "Community involvement is essential to our mission and we look forward to creating lasting connections, providing these individuals the opportunity to serve and be served within their local community." Champions Place utilizes the Medicaid Waiver through Shared Community Support Service. Roommates who qualify have the opportunity share the cost of personal staff. Cost sharing, combined with reasonable rent, provides a cost-effective to living at home while allowing these young adults to be independent.<br><br></div>]]></description>

         <description><![CDATA[<div>CID of San Mateo County provides services to people with disabilities who are trying to become more independent and successful at the post-secondary level. CID helps people to find employment and ensure that they have the required independent living skills to access as much independence in their daily lives, particularly with supervised housing communities. Additionally, they provide access to assistive technology and mental health resources. CID stands for "Center for Independent Living". The most interesting thing I noticed was that this organization will work with people prior to adulthood in order for the person to get the most out of their program at the post-secondary level. </div>]]></description>

         <description><![CDATA[<div>Aspire’s Harry and Jeanette Weinberg Career Academy allows individuals with disabilities to discover and pursue careers based on their interests and strengths. Anyone with a disability and a passion to have a career is eligible to have a career assessment completed by the Aspire team and receive employment services. Aspire focuses on developing specific skills that are needed for jobs that they are interested in. They provide training and coaching that allows individual to be both successful and challenges to allow growth.Aspire provides services from the beginning stages of how to get a job through maintaining the job. They also provide services from transportation training to stress management. They also strive on matching individuals with an employee/work place that will allow both parties to benefit from the relationship. "For the employee, we identify their interests, specific capacities and work environment needs. For employers, we identify skill requirements and what is valued most in an employee. Finding a match, or creating a match, is a collaborative effort between Aspire Partners and Aspire Careers."<br>Aspire allows people with disabilities to received hands on training in a stimulated workplace as well as community workplaces. Individuals get places in jobs that empower them, give them satisfaction, and a higher quality of life.</div>]]></description>

         <description><![CDATA[<div>SEARCH INC is an organization that helps people with disabilities with adult learning services, community life, and supported living services. SEARCH allows for individuals to receive the support they need and want and can provide housing for people with disabilities in the individual's family home, small group homes from 1-8 individuals, to semi-independent settings. Any individual with a disability is eligible to receive services from SEARCH as long as they are motivated to continue to develop their independent living skills. People with disabilities will be assessed and complete questionnaires to determine the best option for the individual. Working in a high school, it would be beneficial to have my students start thinking about short and long term living arrangements. My students indicate living preferences during transition assessments, however having the connection with this organization can allow my students to know that are available to them, and the skills that are required to live in certain settings. SEARCH wants to ensure their is a successful balance of customized supports and opportunities for independence. This organization also encourages community integration and ensures that individuals are connected to the world around them. Finally they also offer supports for people who are not living in their "homes", but need supports while living in their home.<br><br>housing/residential/independent living services</div>]]></description>
