In my opinion, the JADA is a great innovation. With maternal mortality on the rise for numerous factors, I am thankful for an innovation like this one to help mitigate hemorrhaging/postpartum bleeding. When analyzing the postpartum arena, monitoring bleeding is a huge part of the mother's care along with vitals, pain, neurological status, etc. I think innovations such as the JADA are just the beginning of creative and minimally invasive interventions to provide continued safe patient care.

Summary

Postpartum hemorrhage(PPH) is usually due to uterine atony. When there is an abnormal amount of bleeding postpartum the fundus is primarily massaged to help with contracting the uterus and slow the bleeding. Sometimes massaging the fundus is not enough with postpartum bleeding and other measures are necessary to help with decreasing further blood loss. When the estimated blood loss (EBL) is between 1,000-1,500mL medical interventions like an intrauterine device, the JADA, are warranted. The JADA is a 1x use an intrauterine device to assist in the cessation of a postpartum hemorrhage or uncontrollable bleeding of the uterus. The JADA simply is a silicone loop lined with vacuum pores. The silicone loop is inserted vaginally then a cervical seal on the JADA is inflated with 60-120mL of sterile water to help keep the JADA in place. The tubing is then connected to wall suction at 80mmg HG+/-10mmgHG. The JADA will assist in contracting the uterus (just as if a person were palpating/"massaging" the fundus). Blood should then drain into the wall canister. The JADA is not a substitute for extreme measures like MTP or invasive procedures, if there is still continued blood loss. If the JADA experiences "device failure", such as failed vacuum seal or ongoing bleeding it should be removed immediately and other interventions should be implemented to control the bleeding. Removal of the JADA can happen as early as 1.5 hours or later as 24 hours, it depends on the MD. When considering discontinuing vacuum suction this may be done after an hour of use and the JADA should stay in place 30 minutes without suction with a deflated cervical seal before removing it. Vital signs, patient status, quantitative blood loss (QBL) and uterine tone should be considered as well. (Overton, MD et al., 2024).
