The pen is a fountain pen that requires ink, Winston used the fountain pen so "the pages would not be scratched by an ink pen." Winston is putting a lot of care into his writing process because he's been having these thoughts for a while an doesn't know whether his diary wil be carried on into the future of Oceania. 
Winston summarizes traumatic experiences like the bombing of children and also his encounter with O'Brien because he believes he shares his thoughts about the Party.
After recalling Two Minute Hate Winston realizes he had written "DOWN WITH BIG BROTHER". This makes him realize it's only a matter of time before the thought police catches.
