We are accountable for obtaining the correct information and documenting in the correct way and if we are unsure of results we get to check with out mentor or another member of staff so not to just make up a number. ACC= do it under my mentor supervision as well as let my mentor double check if I have got the right measurement
3- Put a cannula 
RES= To know if you are able to do a cannula or not as well as inform your mentor that you are not able to do it
ACC- To inform mentor I have not been trained to undertake the procedure safely.
