    1. Work permit application

    • Needs to be completed by the employer and your parent.
    • The employer needs to complete their portion before we can file the information into the state work permits system.
    • For proof of age, put Birth Certificate since we already have it on file.
    • For school district and building, put Ohio Connections Academy, not the school district you reside in.

    2. Physician's Certificate

    • Needs to be completed by your doctor.
    • We must have both forms in order to process the work permit.

    PLEASE NOTE: If you have not had a physical within the past year, you will need to get one before a work permit can be issued to you.

    Once both forms have been completed, you can send them via webmail, fax, or mail to Mary Basham. Here's the contact information:
