It is done by taking a chisel, along with a blunt force, to split the teeth into sharp points.  Its done without pain killers for the past hundred years. It's is a 4 day religious festival after the month of Ramadan. It's celebrated every year by muslims around the world and its one of my favorit time of the year when family and friends comes together and children are gifted with presents and money.
