Patient is a 27-year-old male with a SCI following a motorcycle accident on palmetto highway 2 months ago. Height= 5'8 inches, Weight= 178 lbs

PLOF: Independent with all activities including all ADL's and while maintaining an active physical lifestyle.

CLOF: Pt. requires Max A with slide-board transfer, Mod A with rolling ans supine to sit. Pt. requires proper set-up assistance to have independence with brushing teeth, bathing, and performing most ADL's. Pt. requires Max A with LE bathing, daily skin checks, and bowel/bladder management. Independent with UE bathing with proper adaptive equipment assistance.

Environmental Factors: Pt. lives in a 2-story home with bedroom on the 1st floor. Pt. has a wife and an 11-month old daughter with no physical support from home at this time, but a brother who can assist occasionally. Pt. worked as a data analyst.

Patient's goals: Pt. would like to be more independent with tasks around the home, especially bathroom use and bed mobility. An ultralight manual power assist wheelchair with mid-wheels.  The wheelchair has a motorized wheel that reduces energy expenditure and overall fatigue through its atomic wheel synchronization. This will be beneficial for this pt. due to decreased arm strength and the goal of wanting to be able to be more independent with ADL's at home and playing with daughter.

This wheelchair also offers quick release wheels for when the pt. begins to integrate driving and traveling in general.
