When I read it was about treating chronic wounds with maggots, I was initially disgusted due to a fear of bugs. But, I became intrigued when results were displayed. I was bothered that I could not find the reasoning as to the physiology<em> </em>maggot therapy works. Nevertheless, I would like to see more research on maggot therapy since the trials had proven it to be effective.&nbsp;I also would have liked for the participants to be interviewed on their thoughts and feelings throughout the process of having maggots help heal their wounds</div>]]></description>

         <description><![CDATA[<div>My initial thoughts while reading the article was along the lines of being slightly disgusted, but also interested from the thought of having maggots on my open wounds. As I continued reading, it fascinated me to learn that treatment with Maggot Therapy resulted in faster development of healthy tissue compared to the conventional method of using Hydrogel. The article was definitely intriguing to read and makes me wonder if we could eventually utilize Maggot Therapy as clinicians in the future. </div>]]></description>

         <description><![CDATA[<div>I did not know what to expect when I clicked the link to an article called Maggot Therapy (MT), but I was intrigued nonetheless. I am always amazed that even with all the medical discoveries we have made there are sometimes methods that have been around us all this time. It was remarkable to see that Maggot Therapy actually showed significant results in many aspects of the wound healing process, specifically the debridement of tissue. Even though the MT did not show substantial data for the disinfection of bacterial growth or the complete healing process, it shows that the healing process is a complex series of events and some of these steps can be "outsourced" to other treatments for better results. </div>]]></description>

         <description><![CDATA[<div>My initial thought after reading the headline, “Maggot Therapy in Wound Healing,” was that this method of intervention seems to be both radical and disturbing. I was genuinely intrigued that someone had the idea to even try this sort of therapy on patients with chronic wounds. After reading this review, I believe it’s worth considering MT in a patients course of action given that the results are faster and more effective than hydrogel dressings. I also believe that maggot therapy needs to be researched extensively before it can be implemented as a standard form of treatment. This review is concluding the effectiveness of MT based on a finite number of studies, and it’s difficult to draw an accurate conclusion. There are a number of different factors that are important to be considered including the species of larvae that are most effective, the use of loose or bagged larvae, as well as the duration of each session. Another major factor to consider is whether patients would be willing to have maggots eating away at their wounds, this may be nauseating for most including myself.&nbsp;</div>]]></description>

         <description><![CDATA[<div>This article was a significantly interesting read. I never knew or heard of using maggots as treatment for chronic wounds. After reading, I learned much about the implications and effectiveness of MT and how much more it could benefit people with chronic wounds compared to hydrogel dressings. However, I also learned despite the many studies that was discussed and analyzed within this article, there is much more research to be done regarding maggot therapy before it can be formally introduced as a standard form of medical treatment. In my opinion, this article could be viewed as a guide on how to properly conduct a research project with the intent to truly compare the effectiveness of MT to hydrogel dressings. This article has done well in critiquing other studies that revolve the effectiveness of MT. It exposes where in the methodology of the experiment needs to be improved on and certain aspects of the treatment that needs to be measured, in order to get more accurate results.</div>]]></description>

         <description><![CDATA[<div>My initial reaction to this reading assignment was one of disgust and discomfort. I had feelings of doubt that something this outlandish would even work, let alone become something commonplace in the medical field. When I read that only five studies met the criteria, it demonstrated that the research is new and not necessarily backed. Continuously, there are various inconsistencies in the studies; duration of treatment, how many larvae to use, and the type of larvae. Moreover, the findings of complete healing were roughly zero across the board. However, every study found that debridement was much faster than when they used hydrogel dressings.</div><div><br></div><div>It seems, to me, that Maggot Therapy (MT) should be used as a beginning of treatment for those suffering from chronic wounds, not as a continuous treatment. After the debridement process is complete other forms of treatment should be used in conjunction, assuming that more research would be done with more established data to back it up.</div><div><br></div><div>As a side note: I can’t imagine many patients complying with this form of treatment unless the benefits truly outweigh the discomfort of their wounds. It seems unclear as to how painful this treatment is. With all this in mind, I’m still hesitant to dive into this form of medical care. The patient and their healing are, of course, the primary goal. On the other hand, if the patient presents with pain from this form of treatment, we should probably reconsider other forms of treatment.<br><br>Leah Klavan</div>]]></description>

         <description><![CDATA[<div>At first I was skeptical of this article, I wondered how physiologically the maggots would aid in wound healing, specifically the disinfecting aspect. As I read the article, I read that there was no significant difference for disinfecting wounds, while most other categories found maggot therapy to be favorable over hydrogel dressings. I noticed there were numerous differences in the five studies that the authors pointed out. The studies they reviewed took place in different countries, with different sample sizes, and with different amounts of wounds per subject. One of the studies also used a different species of fly larva compared to the other four. All of the studies used subjects with different types of ulcers, which I think is important in order to declare the general effectiveness of maggot therapy in comparison to hydrogel dressings. Despite these major differences between studies, I suppose using these articles to make a general conclusion about maggot therapy is useful for clinicians to refer to. I think additional studies should be done to specify the effectiveness towards different types of wounds, as it may not be the most effective treatment for patients with specific types of wounds.&nbsp;</div>]]></description>

         <description><![CDATA[<div>This was a very interesting article to read considering I didn't know that chronic wounds were becoming such a big phenomenon and that maggot therapy studies were being done. I can't even imagine the feeling of live maggots on me.<br>Although the article does seem to show that there are positive effects such as faster reduction of surface area and debridement of wounds and ulcers there are a couple things I am concerned about. One being the reported pain felt by some patients after the maggot therapy. Are the positive effects of maggot therapy enough to justify the use of MT over conventional therapy like hydrogel dressing? This is something to consider since, according to the article, there is no significant effect in using maggot therapy when it comes to completely healing the wound and the time it takes for the wound to heal.<br>In my opinion until more studies are done and better data and results are collected conventional therapy should remain the main practice to combat chronic wounds and ulcers but the venture into using maggot therapy should remain ongoing. Maggot therapy has the potential to help many people.</div>]]></description>

Maggots have been used for 100s of years for wound debridement so it doesn't surprise me that they would see a benefit to using maggots. However, nowadays i wonder if it is necessary as the wound can be cleaned and sterilized by other means. While the study does say that maggots did a superior job to cleaning the wound than hydrogel bandages, patient comfort should be taken into account as many patients may not be comfortable with this idea</div>]]></description>

         <description><![CDATA[<div>My first reaction to this study was, what are we reading? Then looking deeper into the study, it was a very interesting read. I have heard in the past about the use of insects to debride the wound, but never thought of the positives that could come from this. The fact these studies found that tissue regeneration was faster with the maggots compared to the Hydrogel was definitely not what I was expecting to read. The document is a step in the right direction for other maggot studies. I cannot see the use of maggots used in the medical field until it is proven and even that was the case trying to explain that insects are going to be used on the human body might creep people out. I feel like a survey should have been completed after these studies, on patient emotion/expectations and would they do this study again. I will totally be looking out for further studies on this matter and hopefully a solution that might be outside the box be put into practice.&nbsp;</div>]]></description>

         <description><![CDATA[<div>There have been several times in my life where I have been receiving medical treatment and have thought, "What would we do without modern medicine?". Upon reading the title of this article, I assumed that it would help address this question of mine. Prior to this assignment, I never associated the healing of wounds with maggot therapy (MT). But I then realized that there are several parts of the world that don't have access to the medicine that we do, and that people were being forced to be resourceful. This article did a great job highlighting just how resourceful some people can be. I was shocked to see that MT had a positive effect on several of the aspects that were being measured throughout the studies when being compared to hydrogel dressings. Such as the debridement of tissue as well as the healing process as a whole. However I was not shocked to see that one of the adverse effects that were reported was a significant level of pain. This article did an excellent job with highlighting the various differences in each study that led to their different respective outcomes. More research needs to be done prior to implementing MT as a more mainstream means of wound therapy, however based off the studies that were looked at in this article, it is on the right track.</div>]]></description>

         <description><![CDATA[<div>This was a very informative and interesting study. While I found it to be very interesting what maggots can do for wounds I also was shocked to see that this article was published only 2 years ago in 2020. This might be a method that works and works well, but at the end of the day medicine has evolved so much that with modern medicine I believe it is a bit strange to have maggot treatment for wound care. Modern medicine and patient care have come so far. Today it is all about evidence based research mixed with clinical experience, and patient preference, and unfortunately I do not believe that patients would choose to have live animals crawl on them to help their wounds. If this was a life or death situation I believe the study would be sufficient, but just because it is a just another type of wound care I believe patients would rather choose the more practical and traditional methods that they are used to. I did enjoy the read and am fascinated to see that studies such as these are happening and maybe one day one of these studies will be life changing in which case patients will look past the fact that it is untraditional.  </div>]]></description>

         <description><![CDATA[<div>After reading the title of this article, I was already intrigued. I had heard of maggots being used in the medical field before, but never for wound healing. I became even more fascinated as I read the results. The studies found that chronic wounds treated with MT can attain more granulation tissue and a faster reduction in the surface area of the wound. While the MT had amazing effects on wound healing, there were some things that seemed concerning. In two of the studies that were reviewed, pain was reported as a significant side effect of the MT. Additionally, I do not think that many patients would be willing to put maggots on their wounds when there are other, reliable options that seem to work just as well. I definitely think that MT should be considered as an alternative in patients with chronic wounds and (maybe) a primary option for those who are&nbsp; in low resource settings. This was a very interesting read and I’m curious to see where MT will take us in the future.</div>]]></description>

         <description><![CDATA[<div>The article was very interesting. I had read and seen information about maggot therapy for wound healing before. I was bummed they found so many research articles (1600+) and only found five in total that met their criterion. But I found the results promising.&nbsp;<br><br>Due to the limited number of good research articles (level II), the authors warn of their finding's efficacy and validity. Had they included non-English speaking research I think they could have found way more because as they said many other countries could be utilizing this due to the low cost of fly larvae.<br><br>I would feel confident and comfortable utilizing this treatment due to the efficiency of the maggots and the limited possibilities of user error. Overall, I will wait for further research to know for sure whether the effectiveness of this treatment outweighs traditional therapy.</div>]]></description>

         <description><![CDATA[<div><br>Upon first hearing about Maggot therapy, I was reminded of when people have used leeches to cure people. I believe it's still used today to increase blood circulation and break up blood clots. I think we can be quick to grab our pills, ointments, and "modern technology". People can be quick to judge alternative methods but forget that some of it have worked for hundreds of years to keep our ancestors alive. It was interesting to see how well the maggot therapy worked, especially in various types of ulcers. Even though the studies analyzed occurred in different places with varying sample sizes, I think the article was still thorough in explaining its method and discussion.<br><br></div>]]></description>

         <description><![CDATA[<div>The field of healthcare is an ever-changing phenomenon that has no expiration date in the amounts of innovative work it can produce. Though what seems to be an odd form of intervention, Maggot Therapy is deemed to be a beneficial one in the treatment of wounds. However, despite its effectiveness in comparison to the hydrogel dressings, that necessarily does not indicate that all medical practitioners should be utilizing such an approach. First, it is important to have a wholistic perspective when it comes to the utilization of one intervention over the other. Though some patients may be disinterested in the idea of putting maggots on such a sensitive location, others may resort to this method when compared to other man-made interventions. The goal of medical practitioners should not be to let our own personal preferences bias the way we treat patients, however to have that positive impact on them using the methods that are available at our disposal. </div>]]></description>

         <description><![CDATA[<div>I found the information presented regarding MT to be informative and thought provoking, as it analyzed an unconventional yet potentially promising treatment for wound healing and the debridement of non-viable tissue. I found the results to be interesting, as MT facilitated the development of granulation tissue in addition to a reduction in total wound surface area. I feel as though more research may be directed towards MT and its potential benefits as a source of wound treatment. However, the article presented some limitations such as a small amount of studies being included in the systematic review. Further research using MT should seek to involve a larger population while producing similar reliable results. Additionally, another possible limitation regarding the use of MT and its practical applications is the realm of patient preference. While research can point to positive benefits through the use of MT, some patients may decline the intervention simply because it involves maggots. Considering this, evidence based clinicians are inclined to present new clinically relevant evidence as an option to patients while also considering patient values and preferences to make the most effective treatment decisions.</div>]]></description>

         <description><![CDATA[<div>The title of the article did a great job of attaining my attention. I had never heard of maggot's being used for medical purposes, and have always thought of them in a negative light. Although the researchers was only able to use 5 out of the hundreds of article that fit their criteria. The evidence they accumulate from the 500+ participants is a good baseline and gives future studies a good insight to the potential benefits of using Maggot Therapy (MT) in wound healing.<br><br>It was interesting to see that Maggot Therapy had multiple benefits in comparison to  hydrogel dressings. MT was was more effective in debridement and faster at developing granulation tissue. These two important steps in the wound healing process is already a promising result. I can't wait to see what future studies are able to solidify and hopefully this treatment can be used more widespread.</div>]]></description>

         <description><![CDATA[<div>Similar to some of my peers, I had never heard of maggots being used in a medical setting. I usually associate maggots with trash and rotting food, not with wound care. I found it really interesting that Maggot Therapy (MT) was associated with faster rates of granulation tissue generation and increased reduction in wound surface area. I think that this intervention has the potential to help many patients because maggots are easy and cheap to produce. , I wouldn't want MT because of the reported pain during treatment and in general, the concept of having bugs eating my flesh really grosses me out. Maybe this type of therapy could be used for someone who is interested in holistic/alternative medicine practices.</div>]]></description>

         <description><![CDATA[<div>This was a super interesting and thought provoking article, however I&nbsp; have a ton of mixed feelings about it. This systematic review provides insight on how maggot therapy can be utilized to treat chronic wounds more effectively than typical hydrogel dressings. Not only is it efficient, but it is also easily accessible and not very costly. But then again... it requires live maggots eating away at your necrotic tissue. Amazing, but also my worst nightmare. Some of these studies have also shown that pain can be a significant side effect with MT. On top of this, the larvae can be left on the wound for up to four days.... which sounds terrifying once again. This may not be the most glamorous treatment, however it is really awesome that it exists for those who may chose to utilize it. </div>]]></description>

         <description><![CDATA[<div>My first thought when reading the title was "isn't that shown in horror films to make things interesting and disgusting?" As I kept reading the article, I found it fascinating that something that makes me want to turn the other way can heal people. As the majority of the population in America will be 65 and older in a few years, these types of studies are needed. Though this Systematic Review has limitations, it got the ball rolling for future studies. I would like to see studies give the option of covering the area during treatment or blindfolding if that changes the VAS for pain. I believe it could make a difference from people seeing the maggots eat the wound. Also, more sample sizes and a longer duration of healing are needed for future studies. Ultimately, this article was thorough in finding the studies. I would give maggot therapy as an option to my future patients.</div>]]></description>

         <description><![CDATA[<div>Believe it or not I have heard of Maggots eating various bacteria but not out of human wounds. The evidence from this systemic review could not assume a benefit from MT because of low methodological quality and lack of baseline therapy data. Regardless of the results it is very interesting to know that modalities such MT are available, safe and potentially effective to be use.  More research is yet to be conducted concerning maggots and there are various species of maggots have yet to be tested.          <description><![CDATA[<div><strong>&nbsp;</strong>I have never heard of maggot therapy until now. It is very interesting to me that this treatment can be used in any of the stages of wound healing. Also, it is very interesting to me that the maggot therapy technique is used in modern settings and it was used centuries ago. In my opinion it is very promising that Maggot therapy, specifically to treat chronic wounds, saw an increase in granulation tissue and wound surface area was reduced at a quicker rate. It is interesting that although the wound surface area reduced at a quick rate, there was no significant improvement found in healing time. I feel that this is a better alternative to debridement. The maggots are natural and can be a useful anti-infection therapy. I am especially interested to see future studies on the effect of maggot therapy for diabetic wounds.&nbsp;</div>]]></description>

         <description><![CDATA[<div>Jackie Albanese<br>I found this analysis really interesting and insightful. I think the most interesting aspect of this article is the idea that this intervention seems to be viewed as a lesser option and reserved  for developing nations only instead of a viable option across any health care front- including in western nations. I think medicine in developed nations has become very focused on synthetic intervention which has made for some distrust in the medical field due to patients not knowing or trusting what they are taking. If maggot therapy is effective I believe it should be a viable option for individuals looking for a more wholistic way to heal. Giving people options on what interventions are viable and effective gives people autonomy over their health, thus forming a better relationship with their health care providers. </div>]]></description>

         <description><![CDATA[<div>Prior this this class, I have heard of maggots being used to clean wounds before, but that was the extent of my knowledge.  Upon reading this article, I find it very interesting that we can use maggots as an alternative to cleaning wounds. Based on the systematic review, it seems that there is a strong indication for using MT to help with debridement of non-viable tissue and a positive effect on granulation tissue formation. While this may be important for the quality of their wound, it does not significantly hasten the recovery according to the studies when compared to traditional hydrogel dressings. The authors of this review included that the use of MT as an alternative for those with chronic wounds who are not fit for surgery or are in low resource settings considering there were no serious adverse effects for the treatment.  While there is no clear indication that MT should be used to treat wounds for debridement over other practices, it is worth knowing that there is an alternative for wound treatment.</div>]]></description>

         <title>I had never heard of MT as a treatment for wound care before. I found this article really interesting because I find maggots to be disgusting and never thought about  their benefit and what they actually do. I thought that this article was really well written and that it covered all the aspects of a systematic review. I liked that the authors tried to give the whole picture (pros &amp; cons) in regards to the studies done using MT. I would definitely proceed with caution if I were to consider MT as a therapeutic tool. However, I do think that MT needs to be researched more in order to implement it in wound treatment across the US. </title>

It is definitely problematic that so many are suffering from chronic wounds in developing countries and having a remedy like maggot therapy that is so easy to access is a bright spot! I never would have expected that the maggot therapy would be a quicker and more effective means of debridement of non-viable tissue. It is so fascinating that maggot therapy was able to develop quicker granulation tissue and reduced the sizing of the wound surface area compared to hydrogel dressings! Although I am not obsessed with the phenomena of maggot therapy, it is definitely cost effective and more efficient way of wound healing in those with chronic wounds which is something I never would have expected to see be true! </div>]]></description>

         <description><![CDATA[<div>Early on in the article when I read that “the use of MT is on the rise due to its safety, efficacy, and simplicity” I was hooked.&nbsp; I was so intrigued, yet confused, how putting bugs on humans is safe, because in my mind, it seems just the opposite!&nbsp; Something I did not particularly like about the article is that it didn’t go into depth on how MT actually works.&nbsp; Throughout the read, I was wanting to know more information on the physiology of MT.&nbsp; For example,&nbsp; when I read that there was statistically significant faster debridement of slough tissue, but only for the first week of MT, I wanted to know why this faster debridement was only for the first week.&nbsp; What was going on in the microscopic level to cause these changes?&nbsp; Why wasn't there fast debridement of slough after the first week?&nbsp; After I read this article, I was still in a little disbelief that MT was a thing, so I went on to look how this out of the box therapy originated. For any of you who are interested, MT was first reported in Renaissance times when military physicians observed that soldiers whose wounds had become colonized with maggots experienced less morbidity/mortality than soldiers whose wounds had not become colonize.</div>]]></description>

         <description><![CDATA[<div>Prior to this class, I did not know that maggots possessed the therapeutic properties that they do! I found it interesting because I do have a strong fear of bugs and the majority of people do find them disgusting. The article highlighted that the majority of the worlds population struggles with chronic wound healing. As we learned in glass, the gold standard for chronic wounds is hydrogel dressings. This study compared this to maggot therapy via literature review. The review found that the maggot therapy was able to efficiently debride non viable tissue as well as faster development of granulation tissue compared to hydrogel dressings. It was concluded that maggot therapy should be introduced in those with chronic wounds. Personally, I think there needs to be more research done. I also think not every one is a fan of bugs and insects so I do think more research needs to be done to find a more efficient option for chronic wounds that do not involve bugs or hydrogel dressings.</div>]]></description>

         <description><![CDATA[<div>I didn't know maggots could be used for wound therapy until this class. The only bug/animal that I knew could be used for any kind of medical treatment were leeches. As much as I hate bugs, I thought this article was interesting. I think it is a good start, but more research needs to be done before this is a widespread therapy for wound care. The studies that were looked at showed that with MT, chronic wounds can produce more granulation tissue and the wound surface area reduces faster. However, there is no evidence that it can disinfect bacteria or reduce the total time of complete healing. So based off of this, why would I choose it for my patient over another treatment. Additionally, I think it would be hard to convince patients to agree to use MT. Unless they are in severe pain, I don't see many patients wanting to put maggots on their wound.</div>]]></description>

         <description><![CDATA[<div>I have heard of maggots covering infected wounds that are not properly taken care of, however it never occurred to me that the presence of these maggots could actually be a good thing in some situations. The study at hand concluded that maggot therapy (MT) facilitated faster and more effective debridement of non-viable tissue. The study even compared MT to hydrogel dressings and found that MT enabled faster development of granulation tissue and increased reduction in wound surface area. This is an important, intriguing finding, however I think it is important to note that only five studies were used. In my opinion, more research should be completed on this and more studies should be conducted to further confirm if this type of treatment is preferred as opposed to hydrogel dressings or other techniques. Aside from this, I think there are a few pros and cons to MT. Pros would be that it is simple, effective and safe and could be used in settings or locations with less funds and resources allocated towards wound care or patients without other options. Cons would likely be related to patient comfort. I don't know about you all, but the thought of bugs covering a wound of mine in order to heal makes me a little queasy. However, I am sure that if this was the safest option with the highest chance of recovery many people, including myself could be convinced. All in all, I think MT is a solid treatment option to have, and more research should be done.&nbsp;</div>]]></description>

         <description><![CDATA[<div>Similar to most people, when I first heard of maggot therapy it seemed extremely unappealing. Having any bug on me is already bad enough, but having an insect that eats flesh on an open wound sounds terrible. I had heard of it being used before, or similar methods like leeches. However, I thought of this as more of an “older technique” or as a last resort if no other option was available. After seeing the results of this article, though, it seems like it is something that should be considered earlier on and not just as a last resort. The fact that the reviewers found the maggot method showed quicker granulation tissue formation was surprising. It somewhat makes sense that maggots would get rid of bad tissue more quickly and more efficiency, as&nbsp;they have no restrictions. They will just go in and do what they do. I would be interested to see a review that included more studies/participants to see if they get the same results, and perhaps compare it to other debridement methods as well. I think this method is a great option for underdeveloped countries or areas, and I think it is even a good option everywhere else. However, I wonder if patients are more likely to push back when this method is suggested, even if it seems like the best option. </div>]]></description>

I was even more surprised to hear that MT is not just for those in a pinch without a close medical facility, but is actually used in modern hospitals. This method has been around for centuries, yet this article was trying to find the most effective way to utilize them. It's a great example of how some early medical interventions/traditions produced positive outcomes, without the scientific reasoning backing it up. This article argues that MT can be just as effective as modern wound debridement methods such as Hydrogel dressings. Patients should feel confident with MT as a more natural and cost effective option for wound debridement. If I ever need a wound debrided, I would be happy to try modern maggot therapy.  </div>]]></description>

         <description><![CDATA[<div>When I first heard about maggot therapy, I figured it would be an uncontrolled form of debridement, inferior to the specificity of sharp debridement. Not to mention how horrible it sounds! However, after reading the article, I find myself increasingly more open&nbsp;to the idea of maggot therapy being used. Pain being a side effect is not exclusive to maggot therapy. It exists in enzymatic and sharp debridement as well. The Type 2 studies and A level evidence reflected in the systematic review (albeit from 5 sources out of 1600) shows me that maggot therapy can be an efficient and cost effective alternative to debridement, especially in international communities. The biggest barrier to its use seems to understandably be from the discomfort of having live maggots eating your non-viable tissue away for an extended period of time. I get why people would be reluctant, but looking just at the evidence presented, it seems like a promising alternative.</div>]]></description>

         <description><![CDATA[<div>After reading the study, it seems MT could be a viable option for wound debridement in certain cases. The authors of the study mentioned MT had faster healing times compared to conventional methods. However, a lot of the studies had limitations like small sample sizes and the use of many different therapies that may have affected the results.&nbsp;<br>With the older adult population growing in many countries around the world and chronic wounds becoming so prevalent, MT can prove to be an effective tool but more research needs to be done. The current article states there are very few high quality RCT published. I believe with more research, more facilities will adopt the use of MT.<br>I think one of the biggest barriers for the use of MT is the idea of using a bug for treatment, especially on an open wound. I personally would not be in favor of using MT even if the evidence suggests it is the best practice. There are many alternative forms of debridement that can yield similar results. </div>]]></description>

         <description><![CDATA[<div>Before beginning this class and reading this article, I never would have imagined that maggots could have such beneficial therapeutic outcomes. I was definitely intrigued just by the title of this article and immediately wanted to know more about maggot therapy. Upon reading the article, I was blown away by just how beneficial maggot therapy is for the healing of chronic wounds. To read that it actually had better outcomes than some, already extremely successful, therapeutic treatments such as hydrogel dressing was astonishing. The article states that MT allowed for more efficient and faster debridement of non-viable, necrotic, tissue, faster granulation tissue development, and also quicker wound surface area reduction than hydrogel dressings. The only catch was that MT was found to have no effect on disinfection of the wound nor did it have an effect on the healing rate. In my opinion, I do think that more studies need to be done and more beneficial evidence needs to be found to be able to convince people to willingly accept this kind of treatment. Although I think this is a super interesting, effective, and completely safe treatment, I just can't envision many people signing up for maggot therapy as long as there are other treatments still being offered such as hydrogel dressings. </div>]]></description>

         <description><![CDATA[<div>Maggot Therapy is obviously an extremely compelling form of wound therapy. After reading the study, there are clearly benefits to utilizing it, especially when compared to hydrogel dressings. In summary, MT was more effective at accelerating formation of granulation tissue, as well as decreasing total surface area more effectively than hydrogel dressings. However, I think it is important to consider a few factors that are not emphasized as much as they potentially should be in the article. First, no effect was found from MT on disinfection and bacterial management of wounds. Hydrogel dressings, on the other hand, can be an effective way to promote autolytic debridement while providing a barrier to infectious agents. Also, based upon the size of the larva used, patients in a diabetic study reported significant pain during MT treatments (30% of participants). In contrast, hydrogel dressings are non-adhesive and are designed to maintain a moist, pain free healing environment. Finally, how many patients are actually going to WANT MT? It's not to say that MT is not more effective, but the idea of MT may prevent its use clinically, even in situations where it may be more beneficial. MT is a very interesting alternative to standard wound care methods. There are very compelling arguments to be made for its implementation in the healing process of certain wounds. Personally, I will tell future patients about the benefits of MT, although I think it would be a very difficult sell. Ultimately, I think it does deserve respect as a means to accelerating formation of granulation tissue compared to other methods. </div>]]></description>

This article being a systematic review provides even more evidence and has the potential to guide clinical practice on the use of maggots for wound healing, as it can reduce the need for sharps debridement. However, I'm sure patients would share my initial skepticism and may be grossed out by the concept of placing bugs and larvae on their wound and allowing them to eat the necrotic tissue. I feel like this would be a good quality, natural intervention rather than pumping a patient full of synthetic medication and while it may not alleviate pain as much, the healing process would be greatly streamlined.</div>]]></description>

After reading this article its hard to argue that hydrogel dressings and some other methods aren't outdated. Maggot therapy showed to increase most individuals debridement rate as well as rate of developing granulation tissue. While more studies need to be done on maggot therapy, this literature shows that they can be extremely effective at eliminating unwanted tissue. Maggot therapy was shown to have no effect on disinfection of the wound. Overall, after reading this study, I think maggot therapy should be offered more to patients due to it efficiency. while many people may be weary to try this therapy, the ones that do elect to give it a chance will be happy they did.</div>]]></description>

I personally don’t really like insects and the thought of having them eat away my dead tissue freaks me out. However, the use of maggots for wound debridement makes a lot of sense and I was reminded of the time when I saw them eating a dead stray cat on the streets. Although it can be helpful, I believe that patient comfort is also important. Some patients reported pain as a side effect, which I would like to avoid. Some patients might also be turned off by the idea of having bugs on them. However, since there are benefits to maggot therapy, such as faster development of granulation tissue and decrease in wound surface area compared to hydrogel, I believe it should be an option offered to patients if needed.</div>]]></description>

Currently we are all learning how to properly treat a wound and which specific dressings to use according to the type of wound. Something that really caught my attention was the comparison between maggot therapy and hydrogel dressing. Prior to even reading this article, of course depending on the wound, but hydrogel dressing would be helpful for patients and lead to better outcome. Seeing the results now based off this article just shows that maggot therapy is way more effective than hydrogel even though few studies were done. Of course further studies need to be conducted in order to be confident in this type of treatment.          <description><![CDATA[<div>When I initially heard of maggot therapy, I had my doubts. After reading the article, I can see the benefits.  Maggot therapy helps achieve a faster rate of debridement, more effective debridement of non-viable tissue. It also benefits patients by increasing the rate of development of granulation tissue. I can see many situations in which maggot therapy maybe the best method for wound treatment. however, I believe that it maybe harder for patients to accept maggot therapy  over therapies like Hydrogel dressing. Many people have fears of insects or would be uncomfortable with the idea of maggots on their wound. So it would be important to take patients reactions/fears/emotions about maggots into consideration as well as providing good patient education on the therapy. </div>]]></description>

         <description><![CDATA[<div>In the past I have heard of maggot therapy (MT) for wound healing but I did not know how effective it actually was. However, the outcomes of the 5 studies were surprising to me. In all five of the studies it showed that the debridement of the non-viable tissues showed faster results. Growth of granulation tissue was studied in 3 studies and 2 of 3 showed faster results. Overall the use of MT seems very beneficial but complete healing was studied in 3 studies and all three showed no significant difference. Therefore, I believe MT is still a good option however other options like hydrogel are also very beneficial in the complete healing of the wounds.</div>]]></description>

         <description><![CDATA[<div>Originally, I always thought maggot therapy was an old technique that was no longer utilized.&nbsp; I was unaware that maggots did in fact have positive treatment abilities on chronic wounds and is increasingly becoming more applied in the medical field.&nbsp; This specific study dealt with diabetic foot ulcers, pressure injuries, and vascular ulcers.&nbsp; The treatment outcomes were to determine if maggot therapy affected debridement of nonviable tissue, disinfection of bacterial growth, growth of granulation, reduction of wound surface area, complete healing, and duration of healing process.&nbsp; Results illustrated that chronic wounds could acquire more granulation tissue, faster reduction in wound surface area, and debridement of nonviable tissue.&nbsp; However, there was no effect found on disinfection of bacterial growth or duration of healing process when concerning maggot therapy.&nbsp; What I found interesting to learn was that there is more than one strain of medical maggots used to handle wounds such as L. sericata and L. cuprina.&nbsp; Additionally, these two different strains were found to have differing abilities in cleaning nonviable tissue.&nbsp; Learning more about maggot therapy was eye opening and it will be interesting to see whether the use of maggots as wound treatment will continue to grow in the future.</div>]]></description>

         <description><![CDATA[<div>I found the article really interesting and enjoyed learning about this alternative therapy. I had never heard of utilizing maggots for healing before and was intrigued by the whole concept of using insects for modern day medical treatment. Honestly there was a part of me that was grossed out by the thought of MT as I mentally compared it to the ancient practice of bloodletting via leeches, but I also thought it was really cool. I appreciate the concept of using natural methods for healing and it makes a lot of sense to harness the innate behavior of maggots that feed off of dead tissue. I think the paper did a good job at summarizing the articles that were included in the review and found it interesting how there was such variation between them in terms of how the maggots were applied, how many, and the duration of the treatment. I liked how the article concluding by saying that the results "need to be interpreted with caution" as more studies need to be conducted with larger sample sizes and different designs for us to get a clearer picture of how MT really compares to traditional treatment.&nbsp;</div>]]></description>

         <description><![CDATA[<div>Maggot therapy is something I knew of before reading this article. It is not the most pleasant thing to imagine but that does not change the benefits. I thought it was interesting that one of the search terms used was biosurgery. It makes total sense after I thought about it but using bugs for debridement hardly sounded like anything surgical to me at first. I liked the chart on page 6 that showed the summary of outcomes. Two of the studies in the chart said that the maggot therapy was significantly more painful than the alternative therapy. I wondered what the alternative therapy was for these patients. I think the article did a good job at summarizing the research found on maggot&nbsp; therapy. The biggest downfall I seemed to notice was the pain associated with it and the mental barrier because of the larvae.&nbsp;I think this option would need to be discussed very delicately with a potential patient but could be a valid option for a natural debridement. </div>]]></description>

         <description><![CDATA[<div>Dr. Singson's lesson on BioSurgical Debridement was my first exposure to Maggot Therapy. This systematic review definitely helped to supplement my knowledge on the topic. At first, my reaction, which I'm sure was a common theme amongst my peers, was basically that it was gross and I personally would never want to use it. However, after furthering my knowledge on the topic, I actually find it very interesting. In this article I learned that Maggot Therapy is actually more efficient than other types of debridement, and in my opinion I believe this is because its a naturally occurring process, that these maggots will only eat/debride the dead tissue. The scientific evidence provided in this article in conjunction with Dr. Singsons lecture have made me change my mind drastically on maggot therapy since first hearing of this. I also find it interesting that these maggots are sterile, produced in a lab. They are not the same as the maggots we associate with rotting food, and I think thats an important point to get across, especially to patients considering this type of therapy. Initially, I was extremely skeptical but now if I personally ever had a wound that needed debridement, I would absolutely consider Maggot Therapy. </div>]]></description>

         <description><![CDATA[<div>The first time I learned about maggot therapy was actually this semester in my integumentary course. This is a very interesting alternative to other debridement methods. The scientific evidence behind this form of therapy definitely is promising, however, I believe patient preference will hinder further research. Personally, I do not think I would be open to this form of debridement, but I am glad to learn about this as another method to treat patients in need. From what we learned, maggot therapy was relatively painless, however I find that hard to believe, depending if nerve fibers are still intact. Some studies in the article proved that this therapy was painful and I wonder if that was because of different possible injuries regarding depth and possible neuropathy. Innovation is crucial in the field of physical therapy and I am excited to educate myself and keep my mind open for any nuances. </div>]]></description>

         <description><![CDATA[<div>This article has provided me with all of the knowledge I have about maggot therapy and I think it is really interesting. Personally, I did not know maggot therapy existed but it is cool to see how it can stand its ground when compared to the effectiveness of hydrofiber dressings. Both interventions are helpful in healing wounds, but I had no idea that maggot therapy would be so beneficial for decreasing the surface area of the wound and increasing the amount of granulation tissue that is present. I did find it interesting as well that the different studies that were compared all included different methods as to introducing the maggots to the patient. I am curious about what the difference is in the groups that left the maggots in bags versus the groups that directly placed 5-8 maggots per every centimeter of the wound. I am really excited to do more research and further round out my knowledge on this topic.</div>]]></description>

         <description><![CDATA[<div>When I was first introduced to the concept of maggot therapy, the first thought that ran through my mind was "There's no way this is real". I just could not wrap my mind around the idea of putting bugs on an open wound as a form of debridement for a patient. I also couldn't imagine a patient who would be open to receiving this type of treatment. However, after reading the article, looking at the data, and seeing how beneficial it can actually be for wound care, I am now a little more open to it. Knowing that they (maggots) can help increase the amount of granulation tissue that is present and decrease the surface area of the wound were two of the things that helped me to start looking at MT in a different light. I definitely believe that there needs to be more research on this intervention because without very strong evidence completely supporting its use, most patients will not be comfortable/open to this. However, I believe that with proper patient education, more evidence and clinicians advocating for this, we may see a shift in how patients feel about this treatment</div>]]></description>

         <title>I have heard of maggots and leeches being used for injuries but never really explored different studies regarding this topic. Learning about it in detail from the article fascinated me. I was surprised at how the maggots are able to only eat and clear nonviable tissue like slough from wounds while avoiding any of the healthy tissue. While it is an interesting alternative to wound management, I think the biggest concerns would be the duration of healing and pain associated from using MT. I would not want MT done on my wound if it takes longer to heal and if the growth of the maggots would lead to pain (and psychological discomfort). However, I do think MT has its place with wound debridement especially in areas without proper medical supplies. I would think of it as an &quot;organic&quot; solution. </title>

         <description><![CDATA[<div>When thinking of Maggots, I always thought that they appeared when something sustained irreversible damage - like a carcass left in the wild. Throughout our classes and reading this article it is quite astonishing that Maggots actually have the complete opposite effect of what I once thought. The article pointed out astonishing results when compared to the hydrogel and mechanically debrided wounds. Not only did the Maggots prove to debride the wound faster, they reduced the surface area of the wound and increased granulation tissue faster than the other groups. Contrary to my previous belief, it is astonishing that the Maggots are able to be selective debriders better than FDA-approved wound care treatments like whirlpool or surgery.&nbsp;<br>While the evidence that Maggot Therapy is overwhelming, I think its use and effect on the wound care 'community' will not be as vast. For one, many patients likely would not be willing to accept this kind of treatment, due to the lack of comfort knowing that there are live insects on them. This leads me to question the participant in the study who reported more pain while being treated with Maggots than they did from conventional treatment. I believe this may have been a bias based on their knowledge of Maggots.&nbsp;<br>Personally, even after reading this article and going through the different lectures about Maggot Therapy, I am unsure that I would willingly participate, but perhaps one day, when it becomes more widely accepted, more people like myself will become more accepting of this helpful therapy. </div>]]></description>

However, after reading this article, it is incredible how much maggots can affect the healing process of wounds. Even though it is understood that maggot therapy does not affect the entire wound recovery time, it is still an exceptional option especially when surgical intervention is contraindicated. It is interesting that something so simple can help with the wound healing process and be effective in debridement of non-viable tissue. Although I personally don’t see a future in using maggots myself, I do recognize this incredible phenomena that is both effective and efficient.&nbsp;</div>]]></description>

I find it fascinating how these maggots can end up being an effective debriding tool when it comes to would care. considering that it seems like it would be the last thing you would ever want near an open wound. Although its clear that this is effective in debriding necrotic and non viable tissues it was interesting to see that this study did not show significant improvement in time of recovery compared to other studies. Considering the fact that this is such a low risk treatment its definitely something that should be researched further. Being able to provide solid proof of the efficacy of this treatment clinically could definitely help alter the way we treat patients in wound care</div>]]></description>

         <description><![CDATA[<div>This article introduced the concept of maggot therapy to me very well. Before this class if you would have mentioned the use of maggot therapy to me I probably would not have believed it and put it in with other medieval practices like blood letting with leeches to heal all diseases. After reading I was quite shocked how it is still in affect today and especially how effective it is. One misconception I had about maggot therapy is thinking how they would be laying eggs and turning into flies everywhere and thinking that can’t be clean. After learning about maggot therapy  I came to realize how the maggots are actually genetically modified specifically to be effective healers (and not mass fly producers).  Although I do see how incredible and effective this technique is, I believe it is not commonly practiced because of how undesirable the though is of having therapeutic bugs eating you. I do believe that there is a great use for this technique (for those strong enough to stomach it) but perhaps most people would feel more comfortable with a less buggy treatment for debridement. Maybe with more research this common perception could change. I also find it amazing that these maggots only target necrotic and non viable tissues. I have gone to a spa before where tiny fish eat away the dead skin on your feet so maybe I would try out this maggot therapy technique if it was truly the best option. </div>]]></description>

         <description><![CDATA[<div>Apart from that one episode in House of the Dragon, I never considered maggots a viable option in wound debridement. In theory it makes sense that the maggots feed on the dead tissue while leaving healthy tissue alone. Although fascinating, this is something I would want done on me regardless of the evidence due to personal fear of critters. It is worthwhile to further explore this use of biologic debridement since there seems to be conflicting findings for the use of MT over current treatments. This is a topic I am interested in keeping up with though because maybe in the future it will be the standard treatment. </div>]]></description>

         <description><![CDATA[<div>It seems that there are treatments which continue to surprise me. Before reading this article, I thought of maggots as worms which most people do not want near them.&nbsp; The fact that they can be used to debride non-viable tissue quicker than other treatments is something that I find fascinating.&nbsp; Though the results of the article seem to support the use of maggot therapy, I wonder how many people would actually be comfortable with using this treatment on their wounds.&nbsp; I know that I would rather not have worms eating my tissue.&nbsp; I think for this to be a practical form of treatment, patient education is going to be a must.  The patient may need to be convinced that this is the best option to treat their chronic wound and even then s/he may want to try other forms of treatment first.  Although there does seem to be major benefits to MT and according to three studies it does facilitate faster healing, patients may prefer to try other treatments first.</div>]]></description>

         <description><![CDATA[<div>This article on maggot therapy for me was a real eye opener on the potential benefits of maggot therapy. When I thought of maggot therapy prior to this article, I thought of something that would be a treatment from ancient times and definitely not as a viable debridement tool. As I was reading this article, I was pretty impressed at the available evidence there is to support their use in populations that need them. I feel that although the findings of the article were interesting, it does seem like there needs to be a lot more research done to show that the benefits this technique can be generalizable to the public due to the small sample sizes of the studies highlighted. In addition to this, although most people with ulcers may desperate for any treatment that could possibly help, I question how many people would be willing to go through a treatment like this.  </div>]]></description>

However, it also suggests that more comprehensive studies should be performed in order to gain a better understanding of its effectiveness.
