Do you have any concerns since the last assessment?  If mum / dad happy and have no immediate concerns ask permission to proceed with the check.
Then explain the process starting with the physical exam, and how you will be checking various things relating to babies head, body, skin, etc. Top to toe assessment. I would love you be involved in this assessment and as were going through it please feel to ask questions or comment.

As a phN i will be observing for..
Head- size, circumference, Fontanelles, symmetry, flat head syndrome, eyes engaging, conjuntivitis, any redness and swelling, fixing and following, any signs of haemorrhage. Ensure Neck has no restricted movement, that baby can support head when sitting sitting and no head lag. That baby can when on tummy raise head for 15 seconds.
Body- abdomen (signs of umbilical hernia, signs of granuloma)
Genitalia- check for UDD
Spine- check that back flat except sacral temples
Limbs- Feet are trying to stand a little, looking for any deformities, That baby is bringing hands together to the midline and moving all limbs, putting hands in mouth and trying reach, sucking on fist etc, 
Hips- check for DDH
Skin- mottling, puprley hues, birth marks etc.
Length, weight and height- mark on centile chart
document and create any necessary CP's and referrals if needed. Compliment mum on her engagement

Begins to coo- Explain to mum about importance of playing such as peek a boo. If other siblings this could be a great way to involve older siblings by asking them to play with their little brother or sister. Advise mum to do this when changing babies nappy. Refer to mychild book for more ideas.
