  • Its activity is thought to be due to its active thiol metabolite, which inhibits the binding of adenosine diphosphate (ADP) to its platelet receptor which indirectly inhibits platelet aggregation. This action is irreversible.
  • By inhibiting platelet aggregation, blood flows more freely around the body.

  • Assess for drug interactions.  Increased risk of bleeding with aspirin, NSAIDS, heparin, enoxaparin, and other anti-clotting drugs; Omeprazole and other proton pump inhibitors  may decrease efficacy of Clopidogrel;  Feverfew, garlic, ginger, and ginkgo may increase risk of bleeding.
  • Monitor for signs of bleeding (urine, stool, hematoma, epistaxis, petechiae)
  • Monitor CBC and platelet count
  • Establish baseline enzymes and bilirubin levels.
  • Advise patients that excessive intake of alcohol should be avoided with this medication.
  • Advise patients to inform physicians and dentists that they are taking Clopidogrel before any surgery or dental procedure 
  • Discontinue use 5-7 days before surgery
