When people were trying to go to the countryside to flee the chaos of the fever, the roadside security guards wouldn't let anyone who seemed a little sick in. For example, when the farmer was riding with Matilda and Grandfather to the country, the roadside security guards stopped them to make sure nobody had the fever. When Grandfather started coughing (which isn't even a fever symptom), the farmer lied about Matilda and Grandfather, leaving them behind and carrying on with his own life. It really depends on the social class. It seems as if for wealthy classes, women could just stay at home.
