A family member or friend could be sitting there and the next day be dead from the plague. A ship could be coming into the harbor with all dead people besides the people controlling where the ship is going. It would be set back out to be lit on fire to prevent the spread but at that point the damage would already be done. In the photo, you can see a doctor trying to give time to a skeleton, indicating that the person is a dead person walking.The outfit is what doctors wore to protect themselves from the infection.
